Abstract: RESUMO: A taxa de progressão da Hepatite C em portadores crônicos do vírus da hepatite C (VHC) ao estágio de cirrose é altamente variável e os fatores genéticos do hospedeiro que modificam o curso da doença não são bem entendidos. O objetivo deste estudo foi investigar os alelos HLA de classe I e II e os genes KIR e uma possível associação com a progressão da hepatite C e com a resposta ao tratamento. Para tanto, foram selecionados 145 pacientes com presença de RNA-VHC confirmado por testes moleculares, com infecção ocasionada somente pelo genótipo 1 do VHC. As alterações estruturais no fígado foram analisadas usando a classificação METAVIR, e os pacientes foram classificados em dois grupos: pacientes com alterações estruturais ausentes, leves ou moderadas (graus F0, F1, F2); e pacientes com alterações estruturais avançadas, incluindo cirrose (graus F3 e F4). Após a biópsia, os pacientes foram tratados com Interferon-alfa peguilado (1.5ug/ Kg/ dia) e Ribavirina (1g/ pac/dia) durante 48 semanas para eliminação do VHC, os quais foram classificados em respondedores (RVS), não respondedores (NRS) e pacientes que responderam inicialmente, mas recidivaram dentro de 6 meses após término do tratamento (REC). Os pacientes foram tipificados utilizando a metodologia PCR-rSSO Luminex. A análise estatística foi realizada pelo teste exato de Fisher e 2 quadrado com correção de Yates, onde valores de P<0,05 foram considerados significativos. O alelo DRB1*11 e o haplótipo HLA-DRB1*11-DQA1*05-DQB1*03 apresentaram-se associados com proteção ao desenvolvimento de dano hepático severo (P=0,0016; OR=0,23; IC 95%=0,09-0,58 e P=0,0032; OR=0,24; IC95%=0,08-0,64, respectivamente). Em relação à resposta ao tratamento foi encontrada associação entre resposta virológica sustentada e o alelo DQB1*03 (P=0,03; OR=2,4; 95% IC=1,14-5,0). Não foram encontradas associações entre os genes KIR e susceptibilidade ao VHC e desenvolvimento de dano estrutural no fígado. Entretanto, o ligante HLA-C2 foi associado à infecção crônica pelo VHC (P= 0,04; OR= 1,6; 95%IC=1,03-2,52). Conclui-se, portanto, que os alelos HLA-DRB1*11, DQB1*03, o haplótipo HLA-DRB1*11-DQA1*05-DQB1*03 e o ligante HLA-C2 podem influenciar de forma distinta a progressão da hepatite C e a resposta ao tratamento
ABSTRACT: The rate of hepatitis C progression to cirrhosis in individuals with hepatits C virus (HCV) is highly variable and the host factors that modify the course of disease are poorly understood. The aim of this study was investigate the HLA class I and II and KIR genes and their possible association with progression of hepatitis C and response to treatment. For this, were selected 145 patients with RNA-HCV confirmed by molecular tests, infected only by the HCV genotype 1. The liver hepatic lesions were analyzed using METAVIR classification and the patients were classified in two groups: patients without or with mild structural lesions (F0, F1 and F2 degrees); and patients with advanced structural lesions, included cirrhosis (F3 e F4 degrees). After biopsies all patients were treated with interferon-alfa peguilated (1.5ug/ Kg/dia) plus ribavirin(1g/ pac/ dia) for 48 weeks to clearence HCV, which were classified in responders (RVS), non responders (NRS) and patients that in the onset obtained response, but recidivate in the period of 6 months after end of treatment. The patients were typed using PCR-rSSO Luminex. Statistical analysis was performed using the Fisher exact test and Chi-Squares with Yates correction and P<0.05 was considered significant. The DRB1*11 allele and the HLA-DRB1*11-DQA1*05-DQB1*03 haplotype were associated with protection to the development of hepatic damage (P=0.0016; OR= 0.23; IC 95%= 0.09 - 0.58 e P= 0.0032; OR= 0.24; IC 95%=0.08-0.64, respectively). With respect to the treatment response was found association between the DQB1*03 allele and sustained virological response (P=0.03; OR=2.4; 95% IC=1.14-5.0). No association between the KIR genes and susceptibility to HCV and development of hepatic damage. However the HLA-C2 ligand was associated to development of chronic hepatitis C (P= 0.04; OR= 1.6; 95%IC=1.03-2.52). In summary, the HLA-DRB1*11, DQB1*03, the HLA-DRB1*11-DQA1*05-DQB1*03 haplotype and the HLA-C2 ligand seems to influence in distinct ways the hepatitis C progression and the response to treatment
